If you require Before &/or Aftercare on a regular basis, please enter days of the week and hours needed.

Please note that we are required to have current immunization status for all students - please update us after Dr visits.  As of this date, annual flu immunization is required by the State of NJ for all children under age 5 who attend school.

Please update us on any allergy information.  Written permission is required to administer any medication.  Parents whose children use Epipens are required to instruct their child's Head Teacher on administration procedures. 

If any of your contact info, authorized emergency, &/or pick-up contacts have changed please update your student's records.
